refactor(packages/cli): flatten useDevServer — drop lifecycle wrapper
useServerLifecycle was just "everything that isn't the log" — a junk
drawer, not an abstraction. Inline it back into useDevServer. The only
private hook is useActivityLog which has a clear, bounded purpose.

refactor(packages/cli): flatten WatcherStatus and decompose hooks
- Replace _tag discriminated union with flat string status type
- Add onError callback to WatcherCallbacks (separate from status)
- Split useDevServer into useActivityLog + useServerLifecycle private hooks
- Simplify dev-screen.tsx status rendering (no more _tag checks)

fix(packages/cli): handle close during init and unhandled rejection
- closeAll now sets cancelledRef to prevent in-flight init from
continuing setup after close is requested
- cleanup catches server close rejection to avoid unhandled promise

fix(packages/ui): move ts-morph to dependencies for runtime resolution
The externalized import is a top-level ESM static import that executes
on module load. It must be resolvable or the module crashes.
optionalDependencies was wrong — ts-morph is required, not optional.
